Here at Absolute Rehabilitation and Pain Medicine, we love seeing patients! Whether you find us though a friend or online, we always welcome new faces. New Pain Management patients will be required to submit 3 months of chart notes from previous pain management provider and most recent imaging of affected area.
New HRT patients require recent blood work (within 3 months) consisting of a hormone panel for both men and women. If blood work is needed, we can do so in the office. Please call the office to set a time to stop in for blood work. The lab is in-network and takes all insurances. Men must notify their primary care physician and obtain a digital rectal exam each year. Women are advised to continue their monthly self-breast exams and obtain a mammogram and/or a pap smear as advised by their gynecologist or primary care physician. Receiving a PAP smear in our office is a possibility for some patients.
We accept the following insurances for office visits: BCBS PPO, Medicare, Worker’s Compensation, Humana PPO, Railroad Medicare. We do not accept any Medicaid or AHCCCS plans as we are not contracted. Due to not being contracted, we also cannot accept prior authorizations for visits
