Here at Absolute Rehabilitation and Pain Medicine, we love seeing patients! Whether you find us through a friend or online, we always welcome new faces. New Pain Patients
New Pain Management patients will be required to submit the past three months of chart notes from previous/current pain management provider, as well, as the most recent imaging of the affected area (i.e. X-rays, MRI, CT Scans, EMG). An Opioid Agreement must be signed and monthly urine screening must occur. Also, your first appointment includes an evaluation with our in-house Behavioral Health specialist. Don’t worry, this is just to show you are low risk and to help with medication prior authorizations with your insurance company.
Due to the current opioid environment, we have adopted the 2019 Medicare 200 MME guidelines for ALL patients, even those not on Medicare. While we understand and empathize with chronic pain patients, we must follow the guidelines. After all, we’re no good to you if we are not open!
New Hormone Replacement Treatment Patients
Blood work with the needed test is required. Test results must be within the last 3 months. Blood work can be done in or orders can be sent to a lab of your request. Once lab results are received, we will contact you via email. Don’t forget to check out our Specials!

New Wellness Patients
We now offer wellness to help manage your overall health and prevention. We can also do sports physicals. We are continuously adding to our list of services. If you don’t see it, ask! Wellness visits must be on a different day than Pain Management so we have the time to focus on your needs without time restraints.
We have partnered with an in-house L-CSW will not only help support you while receiving opioids in this current environment, she can also help you as much as needed for other concerns, such as but not limited to, depression, anxiety, PTSD. Not a Pain Management Patient? You are still welcome to schedule an appointment. Even better, she is contracted with several insurances!
We accept the following insurances for office visits: BCBS PPO, Medicare, Worker’s Compensation, Humana PPO, Railroad Medicare, Aenta PPO and Aenta Medicare. We do not accept any Medicaid or AHCCCS plans as we are not contracted. Due to not being contracted, we also cannot accept prior authorizations for visits.
